How to refer a client.
To request an appointment for your client, fill in the form further down this page, or call 804-264-0966 and press 3 and leave a message. Either way it reaches our Referral Coordinator.

Please include with your referral
- The name of the person you’re requesting services for
- Your name and/or organization, and whether you’d like a follow-up call
- Contact information where the client can be reached: phone and/or email, noting any preference and whether it’s OK to leave a message
- A brief description of the reason for services (e.g. couples counseling, services for a child and their age, depression, anxiety, stress)
- Any provider preference
